This service is intended to provide wheelchair accessible transportation for persons who are mobility impaired, to and from medical and therapeutic appointments.

Eligibility

The applicant must be a wheel chair user, particularly with a mobile impairment.

Process

1. The applicant visits the Disability Affairs Division Office and requests assistance from the Social Services Representative (SSR).
2. The applicant is interviewed by the SSR to complete the intake form.
3. The SSR then makes a recommendation based on the findings.
4. The applicant is informed of the recommendation made about whether or not they qualify for the service.
